If the untoward medical event has as an outcome: 1. Life threatening 2. Birth / congenital defect 3. Hospitalization or prolongation of existing hospitalization 4. Persistent / significant incapacity 5. Substantial disruption to conduct normal life functions 6. Requires an intervention to preclude permanent damage
