Kettering Home Care is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to provide skilled in-home therapy services under the direction and supervision of a licensed Physical Therapist. In this role, you'll help patients regain strength, mobility, and independence while delivering exceptional care in the comfort of their homes.

The Physical Therapist Assistant (PTA) is responsible for implementing the established plan of care, educating patients and caregivers, maintaining accurate and timely documentation, and practicing in accordance with state licensure requirements, Medicare guidelines, and agency policies.

Qualifications

• Current Physical Therapist Assistant (PTA) license in the state of Ohio.
• Minimum of one (1) year of PTA experience in an acute care setting required.
• Home health experience preferred.
• Strong communication and interpersonal skills with patients, families, physicians, and interdisciplinary team members.
• Working knowledge of current physical therapy best practices.
• Sound clinical judgment and problem-solving skills.
• Strong organizational skills with attention to detail.
• Ability to prioritize tasks and manage time effectively.
• Valid driver's license, reliable transportation, and current auto insurance with the applicant listed as a covered driver.

Key Responsibilities

• Deliver skilled physical therapy services in patients' homes according to the established plan of care.
• Perform treatments and therapeutic interventions delegated by the supervising Physical Therapist.
• Monitor, assess, document, and report patient progress, responses to treatment, and changes in condition.
• Utilize therapeutic exercises, manual techniques, heat, cold, massage, water therapy, and other physical modalities as appropriate.
• Assist patients in improving mobility, function, strength, balance, and overall independence.
• Collaborate with Physical Therapists to achieve individualized patient goals.
• Communicate effectively with physicians, therapists, caregivers, and other healthcare professionals.
• Participate in interdisciplinary case conferences to ensure coordinated patient care.
• Recommend or evaluate prosthetic and orthotic devices and communicate adjustment or modification needs.
• Educate patients and caregivers on:
• Home exercise programs
• Safety and fall prevention
• Proper body mechanics
• Use of mobility aids and adaptive equipment including walkers, canes, crutches, wheelchairs, braces, and prosthetic/orthotic devices
• Promote patient independence while supporting caregiver involvement.
• Complete accurate and timely clinical documentation in accordance with agency policies and Medicare/Federal regulations.
• Document all treatments, assessments, patient education, and care provided.
• Complete requested chart audits.
• Notify scheduling staff of changes affecting patient visits.
• Attend required meetings, case conferences, orientation sessions, and continuing education.
• Assist with onboarding and mentoring new clinical staff.
• Perform other duties as assigned.
