There are two open positions for this role.
The Case Management Coordinator plays a vital role in supporting the case management team by facilitating communication, coordinating care, and ensuring compliance with managed care requirements. This position involves outreach to members, assisting with care coordination, preparing case documentation, and supporting interdisciplinary team functions. The Case Management Coordinator will work closely with case managers, social workers, providers, and interdisciplinary teams to facilitate seamless care delivery. The Case Management Coordinator assists with the management of members in High Risk Case Management, Complex Case Management (CCM), Special Needs Plan Case Management (SNP) and Coordination of Care including Transitions of Care (TOC).

Proficient in Windows, Microsoft Word, and Excel.

At least two years of experience in healthcare field
