Atracare supports a hybrid work model for some Primary Care providers, offering flexibility beyond standard office hours. This allows providers, including those with childcare or other restrictions, to work after hours and on weekends to meet a full-time commitment.

Position Summary

Atracare is seeking highly motivated, board-certified/board-eligible Primary Care Physicians (Family Medicine or Internal Medicine) to join our established team. The ideal candidate will provide continuous, comprehensive medical care to patients above the age of 18, within an outpatient setting. This role emphasizes preventive care, chronic disease management, and acute illness treatment, all while promoting patient education and wellness.

Key Responsibilities: Patient Care and Clinical Management

• Provide comprehensive primary care, including routine check-ups, health maintenance, preventative screenings, and immunizations.
• Diagnose and treat acute illnesses, infections, and injuries.
• Manage chronic conditions (e.g., hypertension, diabetes, asthma, heart disease) through evidence-based practice and personalized treatment plans.
• Order, interpret, and follow up on diagnostic tests, laboratory results, and imaging studies.
• Maintain accurate and timely electronic health records (EHR) documentation.
• Coordinate care with specialists, hospitals, and other healthcare providers to ensure continuity of care.
• Provide appropriate patient education and counseling regarding health promotion and disease prevention.

Practice Operations and Quality

• Participate in quality improvement initiatives, peer review, and continuous professional development.
• Adhere to all relevant clinical protocols, ethical standards, and state/federal regulations (HIPAA, etc.).
• Meet productivity and quality metrics as established by the practice.
• Collaborate with advanced practice providers (NPs/PAs) and clinical support staff.

Qualifications

Required:

• Doctor of Medicine (M.D.), Doctor of Osteopathic Medicine (D.O.), degree from an accredited institution.
• Completion of an accredited residency program in Family Medicine or Internal Medicine.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Current, unrestricted medical license to practice in the State of Delaware.
• Current DEA and CS registration.
• Proficiency with Electronic Health Record (EHR) systems.
• Excellent communication, interpersonal, and team-based skills.

Preferred:

• 3+ years of outpatient primary care experience required.
• Experience with value-based care models (e.g., PCMH, ACO, CCM, RPM, and APCM).
• Fluency in a second language, highly valued, but not required.

Work Schedule

• Typically, Monday - Thursday, 8:00AM-4:30 PM, Friday 8:00AM-12:00, telehealth.
• Opportunity for Alternative Schedules (Saturday, Sunday and afterhours).
• Patient Load: 16 Pt’s per Day. Patient Panel Size less than 950 patients
