Collaborating Physician — Mobile Bedside Musculoskeletal & Pain Care (Part-Time / Flexible) Cookeville / Upper Cumberland, TN

DGM Health pays a fixed collaboration stipend of $3,000–$6,000/month (depending on how many PAs you support) for genuinely light, flexible collaboration on your own schedule: roughly an hour a week of chart review per PA, plus one on-site visit a month. No call, no weekends, no overhead. The stipend is for that light oversight — the real income comes when you treat your own patients at the bedside. You see the patients; we handle all the billing; you earn a competitive, uncapped share of the collections on the care you personally provide.

DGM Health is a clinician-owned mobile practice delivering consultative, bedside musculoskeletal and pain care to residents of skilled nursing, assisted living, and inpatient rehabilitation facilities, anchored in the Cookeville / Upper Cumberland area and serving Middle Tennessee. We're seeking a collaborating physician for a flexible, part-time role built around your schedule.

This role is designed for physicians who want meaningful clinical impact without the overhead: physicians seeking flexible part-time income, physicians early in their careers looking to add earnings, or physicians already working in post-acute settings. We handle scheduling, billing, compliance, and logistics. You collaborate with an experienced PA team and, if you choose, treat your own patients at the bedside.

Responsibilities

• Serve as collaborating physician under a Tennessee collaborative practice agreement (Tenn. Code Ann. § 63-19) for our PA team
• Provide collaborative availability for consultation and periodic chart review, plus a minimum of one on-site visit per month — more often is welcome, entirely on your schedule
• Be reachable by phone or message during clinical hours for occasional questions
• Optionally evaluate and manage your own panel of post-acute patients (musculoskeletal, pain, functional and rehabilitative needs); DGM handles billing for the services you provide
• Where trained, perform or oversee bedside ultrasound-guided musculoskeletal injections
• Coordinate with facility care teams

Qualifications

• MD/DO with active, unencumbered Tennessee license (or willingness to obtain)
• Board certification in Physical Medicine & Rehabilitation (physiatry) or Pain Medicine
• Post-acute, SNF, or geriatric experience a plus
• Comfort with the post-acute/SNF population and musculoskeletal and pain management
• Current DEA; Medicare-enrolled or eligible
• Ultrasound-guided musculoskeletal injection skills a plus

Compensation & Structure

• Fixed monthly collaboration stipend, $3,000–$6,000/mo depending on the number of PAs you support — roughly an hour a week per PA
• Earn a competitive, uncapped share of collections for patients you personally treat
• Flexible 1099 engagement

Physical Requirements

• Prolonged standing, walking, and sitting during patient care and documentation
• Travel between facilities across the coverage area
• Ability to lift and assist with patient positioning, up to 50 lbs
• Possible exposure to bloodborne pathogens and other clinical hazards
• Compliance with facility PPE and vaccination requirements

Location: Cookeville / Upper Cumberland, TN — mobile/bedside, with remote collaborative availability between visits.
