Elevation Rehab proudly provides in‑house outpatient rehabilitation services to Assisted Living Facilities in Rocky Mount, NC 27801. Our mission is to deliver exceptional, patient‑centered care that helps individuals restore mobility, reduce pain, and regain confidence in their daily activities.

We are seeking a dedicated and compassionate Licensed Physical Therapist (PT) to join our growing team. This role is ideal for a clinician who values meaningful patient relationships, enjoys working with older adults, and thrives in a collaborative, supportive environment.

Physical Therapist Duties

• Conduct comprehensive evaluations of strength, mobility, balance, gait, and functional movement
• Develop individualized treatment plans tailored to patient goals and functional needs
• Provide therapeutic interventions including strengthening, balance training, gait training, and pain‑management techniques
• Educate patients and caregivers on home exercise programs, safety strategies, and mobility techniques
• Collaborate with a multidisciplinary team to ensure coordinated, high‑quality care
• Monitor patient progress and adjust treatment plans as needed
• Maintain accurate documentation, including evaluations, progress notes, and treatment outcomes

Physical Therapist Qualifications

• Doctorate or Bachelor’s/Master’s degree in Physical Therapy (for PT)
• Valid North Carolina state license as a PT
• Experience working with adult and geriatric populations preferred
• Strong understanding of anatomy, physiology, kinesiology, and evidence‑based therapeutic interventions
• Excellent communication and interpersonal skills
• Ability to work independently and collaboratively within a care team
• Compassionate, patient‑centered approach to therapy
