Position Title: Care Manager (experience within FCM, CANS Assessment and POC Required)
Department: Sparx Services Care Management agency
Reports To: Care Management Supervisor / Program Director
Status: Full-Time
Salary Range: $4,000 – $5,000 per month (based on experience and qualifications)
Work Schedule: Hybrid – Required in-office once per week; remainder remote Position Summary: The Care Manager at Sparx Services Care Management Agency is responsible for delivering personalized, strength-based care coordination services to children and youth (Up to age 21) enrolled in the Health Home Serving Children (HHSC) program. This includes organizing, implementing, and monitoring a comprehensive care plan in collaboration with families, service providers, and community supports.
Key Responsibilities:
1. Comprehensive Care Management
- Conduct and regularly update the CANS-NY assessment
- Develop, implement, and revise the Plan of Care (POC)
- Coordinate care team meetings and gather input from multidisciplinary providers
- Monitor progress on individualized goals and service outcomes
2. Care Coordination & Health Promotion
- Schedule and facilitate appointments across behavioral, medical, and educational systems
- Promote wellness initiatives and preventative care
- Communicate with families and providers to ensure service alignment
- Track medication adherence and other health metrics
3. Transitional Care Support
- Support transitions (e.g., hospital discharges, program transfers)
- Ensure continuity of care and timely updates to the care plan
- Provide post-discharge follow-ups and system navigation
4. Family Support & Advocacy
- Advocate for families in education, legal, and social service systems
- Provide caregiver education and emotional support
- Encourage family involvement in treatment planning and service access
5. Community & Social Support Referrals
- Connect families with resources such as housing, benefits, and food access
- Assist with applications for Medicaid, SSI, and other public benefits
- Track referrals and service linkages in documentation systems
Documentation & Compliance Requirements:
- Maintain accurate and timely records of all services provided
- Submit monthly Billing Support Questionnaires (BSQ) and other required reports
- Ensure all required documentation (e.g., consents, CANS, POCs) is current and uploaded
- Comply with HIPAA and other state-mandated confidentiality regulations
- Participate in internal audits and quality assurance initiatives
Qualifications:
- Bachelor’s Degree in Social Work, Psychology, Human Services, or a related field (required)
- At least 1 year of experience with youth facing mental health, medical, or developmental challenges
- Completion of mandatory trainings:
- HIPAA
- Mandated Reporter
- Strong organizational and interpersonal skills
- Ability to work independently while collaborating with remote and field-based teams
- Bilingual preferred but not required
Work Environment:
- Hybrid role with in-office attendance required once per week
- Remote case management, documentation, and communication throughout the week
- Field visits as needed for assessments, family meetings, or provider coordination
