GENERAL JOB SUMMARY:
West Valley Community Services is a nonprofit organization that has been providing safety net services to low-income and homeless individuals and families in the West Valley region of Santa Clara County for more than 52 years. West Valley Community Services is currently hiring an HPS Program Coordinator to assist with the operation of the Homelessness Prevention System program by providing case management for clients. The coordinator is also responsible for workshops, outreach, and special projects as needed. The Coordinator will also work with the outreach committee to identify areas of need and conduct monthly outreach. This position requires a candidate to work in the WVCS office in person full-time.
Duties and Responsibilities:
- Conducts comprehensive Housing Problem Solving Assessments (HPAT) to evaluate housing barriers, eviction risk, financial stability, household needs, and other factors to determine eligibility for Homelessness Prevention Services (HPS), both in person, virtually and by phone. .
- Maintains a flexible caseload of approximately 30 households, providing housing stabilization, eviction prevention, problem-solving, and intensive case management services to households at imminent risk of homelessness.
- Develops support systems that address barriers to housing retention by coordinating financial assistance, supportive services, community resources, and long-term stabilization strategies.
- Provides housing navigation by connecting households to appropriate internal and external resources, including financial assistance, legal services, employment supports, benefits, and other resources that promote housing stability and prevent homelessness.
- Receives, monitors, and coordinates inbound and outbound HPS referrals, including screening households, scheduling Housing Problem Solving Assessments (HPAT), and ensuring timely follow-up.
- Manages the WVCS appointment system for HPS participants, including scheduled appointments, same-day requests, and walk-in households seeking eviction prevention.
- Assists with planning and implementing community outreach, housing resource workshops, presentations, and engagement activities to increase awareness of homelessness prevention services and connect households to available resources.
- Maintains HPS client-facing educational materials, outreach supplies, and resource information, ensuring counseling rooms, lobbies, and outreach events are fully stocked with current homelessness prevention and housing stabilization materials.
- Manages HPS client-facing outreach materials and ensures proper replenishment of client lobby and counseling rooms.

QUALIFICATIONS
- Interest in social work or social services.
- Ability to work with people from diverse backgrounds and/or with language barriers. Bilingual highly desirable.
- Interpersonal skills with the ability to communicate in individual and group settings.
- Ability to embrace WVCS’ mission to provide basic human needs with compassion and in a dignified environment.
- Valid California driver’s license/ID and insured automobile preferred.

Salary Range:
- $24.95-$29.00/hour ($51,833-$60,320/year, paid semi-monthly
Job Type:
- Full-time, Non-Exempt, Permanent (40 hours per week)
Schedule:
- Standard 8-hour day shift.
- Monday-Friday, 8am-5pm & (1) 11am-8pm Thursday shift per month with occasional evenings and weekends as required by program needs
