Position Overview
TAG MedStaffing is hiring an Outreach Program Coordinator (LMSW) to support community-based outreach programs serving individuals experiencing homelessness in Brooklyn and Queens. This leadership role oversees daily field operations, supervises outreach teams, coordinates community coverage, and promotes housing-focused, trauma-informed services.
The ideal candidate will have experience leading multidisciplinary teams, responding to crisis situations, and collaborating with community organizations and public agencies to connect individuals with housing, behavioral health services, and other essential resources.
Responsibilities
- Supervise outreach staff and oversee daily field operations
- Develop staff schedules and coordinate field coverage
- Guide outreach efforts, assess client needs, and facilitate connections to housing and community resources
- Provide leadership during crisis situations and support complex client cases
- Monitor documentation, reporting, and program performance to ensure compliance with organizational and contractual standards
- Partner with community organizations, shelters, and public agencies to support care coordination and housing placement initiatives
Qualifications
- Master's Degree in Social Work required
- Active New York State Licensed Master Social Worker (LMSW) license required
- Valid driver's license in good standing required
- Ability to obtain required field credentials within the first three months of employment
- Minimum of 3 years of supervisory experience
- Knowledge of homelessness services, trauma-informed care, crisis intervention, harm reduction, motivational interviewing, and housing-focused service models preferred
- Proficiency with Microsoft Office (Word, Excel, Outlook, and Teams)
- Strong leadership, communication, organizational, and crisis management skills
- Bilingual English/Spanish preferred
