Vanguard Behavioral Health is seeking a compassionate, organized, and motivated Case Manager to join our growing behavioral health team. This position plays a vital role in supporting individuals with substance use disorders and co-occurring mental health conditions by connecting them with community resources, coordinating care, and helping remove barriers to recovery.

Responsibilities

• Coordinate client care throughout the continuum of treatment.
• Conduct case management needs assessments.
• Develop and maintain individualized service plans in collaboration with the clinical team.
• Assist clients with accessing housing, transportation, employment, medical care, insurance benefits, and community resources.
• Coordinate referrals to outside providers and community agencies.
• Maintain regular communication with treatment providers, referral sources, probation officers, family members, and other supports as appropriate.
• Monitor client progress and assist with discharge and aftercare planning.
• Complete accurate, timely, and compliant documentation in the electronic health record.
• Participate in treatment team meetings and collaborate with therapists, medical providers, and support staff.
• Advocate for clients while promoting independence and self-sufficiency.

Qualifications

• CCSS required, CPSW, LSAA preferred.
• Experience working in behavioral health, substance use treatment, case management, or community mental health preferred.
• Strong organizational, communication, and problem-solving skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of community resources throughout New Mexico is preferred.
• Experience with electronic medical records is a plus.
• Valid driver's license and reliable transportation preferred.
