Vanguard Behavioral Health is seeking a compassionate, organized, and motivated Case Manager to join our growing behavioral health team. This position plays a vital role in supporting individuals with substance use disorders and co-occurring mental health conditions by connecting them with community resources, coordinating care, and helping remove barriers to recovery.

Responsibilities
- Coordinate client care throughout the continuum of treatment.
- Conduct case management needs assessments.
- Develop and maintain individualized service plans in collaboration with the clinical team.
- Assist clients with accessing housing, transportation, employment, medical care, insurance benefits, and community resources.
- Coordinate referrals to outside providers and community agencies.
- Maintain regular communication with treatment providers, referral sources, probation officers, family members, and other supports as appropriate.
- Monitor client progress and assist with discharge and aftercare planning.
- Complete accurate, timely, and compliant documentation in the electronic health record.
- Participate in treatment team meetings and collaborate with therapists, medical providers, and support staff.
- Advocate for clients while promoting independence and self-sufficiency.
Qualifications
- CCSS required, CPSW, LSAA preferred.
- Experience working in behavioral health, substance use treatment, case management, or community mental health preferred.
- Strong organizational, communication, and problem-solving skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Knowledge of community resources throughout New Mexico is preferred.
- Experience with electronic medical records is a plus.
- Valid driver's license and reliable transportation preferred.
