General Responsibilities

The Community Living Team Coordinator (CLTC) supports the Community Living Coordinator (CLC) to oversee and coordinate Supported Living Services (SLS) and 24-hour residential services for those on the DD-waiver living in the community or in Host Home (HH) and Family Caregiver (FCG) settings. This includes individuals receiving Medicaid HCBS-DD and HCBS-SLS funding, as well as private pay and state funded services across all counties. The CLTC is responsible for assisting the CLC with program management, billing, utilization review, quality assurance, and compliance with all relevant regulations while emphasizing person centered services. The CLTC trains and supports Direct Support Professionals and contracted service providers (HHP & FCG). Responsible for tracking staff trainings and working with the CLC and Human Resources Department to make sure staff files meet regulatory standards. This position works closely with the Community Living coordinator, agency nurses, Case Management Agency (CMA), families, Direct Support Professionals (DSPs), and contracted service providers (HHP & FCG) to maintain high-quality, person centered services.

Education and Experience Requirements

1. High School Diploma or GED required

2. Associate’s degree with related course work in human services or commensurate experience preferred.

3. Working understanding of the IDD system, Medicaid HCBS-Waivers, and associated rules and regulations.

Skills Requirements

1. Proficient knowledge of Microsoft Office including Excel, Word, Publisher, and PowerPoint and an aptitude for online platforms like Google Suite, ADP, and Therap.

2. Excellent interpersonal communication skills to work effectively with a diverse range of individuals and personality types.

3. Strong organizational skills to support staff and program requirements.

4. Ability to navigate unexpected or new situations to guide individuals in services, family members, and Direct Support Professionals using professional judgement.

Other Requirements

1. Must be at least 18 years of age.

Administrative Responsibilities

1. Maintain ongoing communication with the CLC including weekly updates and meeting at minimum of once per month.

2. Provide day-to-day support to DSPs and contracted service providers.

3. Provide training on agency, program, and person specific information to DSPs and contracted service providers.

• After any in-service, conference or training, ensure participants who attended are documented on the Inservice Training Form, submit to Human Resource Coordinator.

4. Provide feedback to CLC so they can address performance concerns via retraining, performance requests, and conference statements in alignment with Horizons Adult Program Performance Improvement Procedures.

5. Provide written input on staff performance to CLC to be included in annual performance reviews.

6. Discuss needs to be addressed in staff meetings/create agenda for meetings and schedule staff meetings monthly or as needed.

7. Complete and update program summaries, with DSP and contracted service provider input, to ensure accuracy of information and individual.

8. Review and update Emergency Data Forms as needed, or annually at minimum.

9. Review each individual program periodically to ensure that services are being provided in accordance with the IP and are having the desired effect. Reviews shall include visits to sites where services or supports are provided and, whenever possible, observation of the actual provision of services.

• Provide the CLC with a summary of the findings of each service review and make recommendations on PAR amendments as needed.

• Formal reviews shall be, at a minimum, conducted on a quarterly basis. If more frequent review is necessary to ensure program effectiveness and coordination, the review will be conducted at a frequency as determined appropriate for the specific program.

10. Create new ISP programs or make updates to current ISP programs, as needed for changes in individual goals or services.

11. Create new ISP programs for new enrollments.

12. Assist with creating a set, consistent schedule for staff for individuals enrolled in the Community Living Program.

• Monitor the changes in DSP schedules to assure coverage meets client needs, not just staff personal needs, unless otherwise approved by the CLC.

13. Work with CLC to complete monthly billing

• Enter attendance billing through Therap for individuals receiving HCBS-DD IRSS services.

• Review ISP notes for individuals receiving HCBS-SLS services to ensure accuracy of coding and provide feedback to DSPs to ensure understanding of billing categories, as needed.

• Transfer SLS service billing hours from ISP notes to approved billing form for approval by the CLC.

Regulatory & Compliance Responsibilities

1. Support the CLC to ensure compliance with federal, state, and local regulations and

organizational policies. This includes:

• Colorado Department of Public Health and Environment (CDPHE)

• Colorado Department of Healthcare Policy and Financing (HCPF)

• Federal Department of Housing and Urban Development (HUD)

• Department of Labor (DOLA)

2. Ensure Mandatory Reporting Guidelines are followed for all suspected incidents of

mistreatment, abuse, neglect, and/or exploitation (MANE).

3. Develop and update service plans, hold interdisciplinary team (IDT) meetings, and implement ISSPs ensuring proper documentation and follow-up.

4. Complete statutorily required monitoring to ensure program effectiveness and individual safety, including formal quarterly or six-month reviews, as required.

5. Support the CLC to ensure compliance with Medicaid HCBS Waiver and SLS State Funding enrollment requirements.

6. Support the CLC to ensure familiarity with program regulations and participate in state meetings or trainings on regulatory changes.

7. Support the CLC to prepare for and responds to third party surveys including CDPHE, and Horizons’ internal surveys.

8. Support the CLC to provide information, as required, for Human Rights Committee (HRC) review for all Rights Modifications, Safety Control Procedures, Emergency Control Procedures, and/or Psychotropic Medication.

Client and Family Services & Coordination

1. Serve as an additional point of contact for individuals receiving services (IRS), families, and contracted service providers.

2. Cover open shifts, as necessary, to ensure that services are provided in accordance with the individual service plan.

3. Assist the CLC with new enrollments, ensuring that all paperwork is completed and services begin in a timely manner.

4. Support the CLC to ensure that service provision aligns with individual service plans (IPs) and client needs.

5. Promote wellness, nutrition, and healthy lifestyles for individuals through training and protocol implementation.

6. Check monthly with DSPs about individuals’ apartments to determine if there are any special needs or repairs, and assist with arrangements.

7. Keep a current list of the clients’ landlords and the phone numbers to provide to DSPs.

8. Work with DSPs to ensure clients have a First Aid Kit placed in their apartment and that the kit is replenished when necessary. This is completed during client residential monitoring.

9. Maintain open and professional communication with case managers, families, direct support professionals, and contracted service providers to ensure seamless service delivery.

• Support the CLC to complete Program Summaries and PAR Requests prior to the annual service plan meeting.

• Track utilization and make recommendations to the CLC for PAR Amendments.

• Work cooperatively with members of the IDT in order to ensure that everyone is informed of proposed changes in client programs, residential status, behavioral issues, communicating with families of clients, and identified needs on a timely basis.

10. Ensure that individual program files, including Therap records, are up-to-date and in compliance with regulatory requirements.

11. Travel within Horizons’ service area, as necessary.

12. Participate in the agency on-call rotation, as scheduled.

Medical and Health Coordination

1. Work with the CLC, Nursing Department, direct support professionals, and contracted service providers to coordinate medical appointments and ensure proper health monitoring for the individuals on the HCBS-DD waiver.

2. Support the CLC to ensure annual physical, dental, and vision exams are completed for individuals receiving services.

3. Monitor medication administration procedures and provide ongoing health-related support.

• Ensure that counselors are following the new medication procedures by checking

medication labels and compare to medication sheets during monitoring.

• Ensure the drug disposal procedure is being performed when necessary.

• Ensure GER reporting is completed as required for medication errors.

4. If an individual in services is receiving therapy ordered by an approved provider (OT, PT, SLP, etc) that is delivered by a DSP or contracted service provider ensure that required training and competency is evaluated and documented by the approved provider annually, unless more frequency is provided.

• Schedule new DSPs or contracted service providers to receive training and competency evaluation before therapy is provided.

5. Respond to medical emergencies, as needed.

6. Support the CLC with the development of individual care plans, as needed, to address ongoing medical needs or behavioral concerns with support from the Nursing Department or Behavior Support Coordinator, or outside ABA therapists.

Agency Specific Duties

1. Maintain confidentiality of individuals receiving services and employees in alignment with HIPAA and employment laws.

2. Advises the CLC on current and anticipated matters which may affect individuals in the Community Living Program.

3. Works with the Resource Development Coordinator to:

• Accurately record and report in-kind donations made to the Community Living Program.

• Train and accurately record hours of service provided by volunteers.

• Report on any grant/donor funds in accordance with the requirements of the grantor or donor.

4. Represent a positive image of Horizons.

5. Participate in and organize agency trainings, as needed or requested, to ensure agency alignment and regulatory compliance.

6. Ensure that office spaces and agency properties present a positive image.

7. Ensure that all work spaces are safe for individuals, staff, and contracted service providers.

8. Ensure that all program and/or agency vehicles utilized by the Community Living Program are in good repair. Communicate maintenance needs to the Office Coordinator or the Human Resources Coordinator.

9. Adhere to and model agency policies, procedures, and core values.

10. Other duties assigned by the Community Living Coordinator.
