FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is seeking a compassionate Certified Nurse Practitioner, is seeking a compassionate Certified Nurse Practitioner to support patients in its Palliative Care Advanced Illness Management program. This role works closely with patients, families, caregivers, and health care professionals to promote comfort, dignity, and quality of life through thoughtful assessment, care planning, and coordinated care.
Ideal candidates are compassionate, team-oriented professionals with strong communication and social interaction skills. Applicants must be able to analyze patient needs and take appropriate action. Experience working with end of life care and/or healthcare setting preferred.

- Assess new FAIRHOPE Advanced Illness Management Palliative patients and develop individualized plans of care.
- Re-evaluate and revise plans of care as patient needs change.
- Triage calls and coordinate patient care delivery.
- Provide preventative, therapeutic, and rehabilitative nursing care.
- Monitor changes in patient condition and communicate updates with families, caregivers, and physicians.
- Support continuity of care by collaborating with patients, caregivers, families, and health care professionals.
- Administer medications and treatments as prescribed and prescribe medications as appropriate.
- Complete face-to-face encounter visits according to regulatory guidelines.
- Seek and obtain sufficient information necessary to effectively perform agency operations in the health care setting where assigned.
- Document services, communication, and care updates electronically.
- Maintain HIPAA compliance at all times.
- Graduate of an accredited school of professional nursing, Master of Science in Nursing required.
- Is currently licensed as a Registered Nurse through the State Board of Nursing.
- At least one year of general nursing experience, with current home health, medical, surgical, or critical care experience.
- Is self-directed with the ability to work with little or no supervision. Has excellent coordination and communication skills.
- Possess current Basic Life Support (BLS) certification.
- An individual with the ability to identify areas for improvement and take necessary action.
- Demonstration of excellent communication and interpersonal skills, as well as a caring attitude.
- Computer skills required.
- Meets FAIRHOPE's credentialing standards and requirements.
- Physical Setting: Various healthcare settings. Some driving is required.
- Work Location: Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties. Main office location is located in Lancaster, Ohio.
