We believe that in-person services are the best way to serve our clients, both directly and through essential operational support. This position requires in-person attendance with a team at 1 Brooklyn clinic location with some travel in support of the CCBHC Project.

Certified Community Behavioral Health Clinic (CCBHC) Core Services includes:

• Crisis mental health services including 24-hour mobile crisis teams, emergency crisis intervention, and crisis stabilization; Screening, assessment, and diagnosis including risk assessment; Patient-Centered treatment planning or similar processes, including risk assessment and crisis planning; Outpatient mental health and substance use services; Outpatient clinic primary care screening and monitoring of key health indicators and health risk; Targeted case management; Psychiatric rehabilitation services; Peer support, counselor services, and family support services; and, Intensive community-based mental health care for members of the armed forces & veterans.

SUMMARY: The CCBHC Psychiatrist will work with a team of professional social workers, clinicians, case managers and peers to ensure integrated care for adults experiencing a serious mental illness and/or substance abuse disorder, and a chronic health condition. As a member of the client care team, the Psychiatrist will establish a therapeutic relationship with the client to facilitate the assessment, diagnosis, prescribing of medication, monitoring client progress, and adjusting treatment to better stabilize MH, SU and health needs of clients living in the community for an integrated and person-centered care. To be effective, the Psychiatrist will have the demonstrated ability to balance independent clinical decision making with appropriate consultation with other providers and work with the CCBHC multi-disciplinary team to engage, establish and achieve appropriate treatment goals.

JOB DESCRIPTION:

• Perform evaluation and assessment of clients requesting substance abuse treatment who may have a mental health disorder and/or chronic health condition/s.
• Perform medication evaluation to establish and prescribe medication treatment as part of an integrated treatment approach.
• Perform on-going pharmacologic treatment and monitoring of the client’s experience with the medication and barriers to medication adherence.
• Participates in incident review as part of a multidisciplinary team, as appropriate and needed.
• Actively participates in treatment planning for an integrated person centered approach and performs updates as required by program policy and standards.
• Participates in case conferences with the multidisciplinary team, as needed.
• Perform accurate diagnosis of clients and collaboratively work with CCBHC team to improve client’s progress toward treatment goals.
• Monitoring client to check progress and adjust the treatment plan as needed.
• Thoroughly documenting each client's diagnosis, symptoms, medication, treatment effectiveness, etc.
• Providing patients with emotional support and strategies to help manage their disorder.
• Researching new developments in psychiatry and mental health. Incorporated evidence-based practices including cognitive behavioral therapy and self-management.
• Train staff, as needed, on management of substance use related health problems and specifically on symptoms of withdrawal and opioid use.
• Provide consultation on MAT to clinical staff across sites.
• Notify Infection Control Coordinator immediately of suspected or confirmed communicable diseases, which can be transmitted through routine shelter contact.
• Psychiatric evaluations and medication management documentation shall meet all stakeholder, payer, and regulatory expectations and include clinically appropriate diagnoses and recommendations.
• Utilizes evidence-based practices as appropriate for the individual client and program philosophy.
• Prescribe buprenorphine-naloxone / buprenorphine to patients who are enrolled in the MAT program and who are in compliance with treatment contract.

EXPERIENCE:

• Extensive knowledge of the DSM.
• Expertise in diagnosis and treatment of substance use disorders.
• Extensive experience with MAT.
• Knowledge of the intersection of mental health, substance misuse and chronic health conditions.
• Experience with adults with mental health and/or substance abuse needs.
• Experience with performing evaluations, prescribing and medication monitoring.
• Enthusiasm and self-motivation regarding identifying client needs.
• Familiar with crisis assessment tools and techniques.
• Strong working knowledge of general psychiatric principles and practices.
• Well organized with strong time management skills.
• Excellent written and verbal communication.
• Able to work independently and as a member of a team.
• Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues.
• Proficient with word processing, electronic health records, and electronic communications.
• Demonstrated competency in written and verbal skills.
• Able to meet program standards in documenting in client records.
• Ability to work with challenging clients.
• Comfortable with community outreach and home visits.
• Comfortable and competency working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities.
• Bilingual skill in a language other than English valued.

QUALIFICATIONS:

• Doctor of Medicine from Accredited Medical School,
• Minimum 3 years psychiatric experience under supervision in a residency program from an accredited medical school and completion of an approved psychiatric residency
• NYS Board of Psychiatry certification and license.
• Current DEA registration
