About Mindful Medicine MDs

Mindful Medicine MDs is a growing, patient-centered medical practice dedicated to

providing exceptional care for older adults throughout California. We are seeking a

compassionate and experienced California-licensed Physician Assistant (PA-C), Family

Nurse Practitioner (FNP), or Internal Medicine Nurse Practitioner (NP) to join our team

and provide remote Transitional Care Management (TCM) services to patients recently

discharged from Skilled Nursing Facilities (SNFs).

This is an excellent opportunity for a provider who enjoys building meaningful relationships

with geriatric patients while working from the comfort of their home with a flexible

schedule.

Responsibilities

• Conduct Transitional Care Management (TCM) telehealth visits for patients recently

discharged from Skilled Nursing Facilities.

• Perform comprehensive post-discharge assessments.
• Review hospital and SNF discharge summaries, medications, and treatment plans.
• Develop and document appropriate care plans.
• Identify barriers to recovery and help reduce avoidable hospital readmissions.
• Complete timely and accurate documentation in the electronic medical record.
• Deliver compassionate, patient-centered care while maintaining the highest

standards of professionalism.

Qualifications

• Active California Physician Assistant (PA-C), Family Nurse Practitioner (FNP), or

Internal Medicine Nurse Practitioner (NP) license in good standing.

• Current DEA registration.
• Experience working with geriatric, primary care, internal medicine, family medicine,

or post-acute patients is strongly preferred.

• Excellent communication and interpersonal skills, particularly when speaking with

older adults and their families.

• Compassionate, empathetic, and patient-focused approach to care.
• Strong clinical judgment and organizational skills.
• Comfortable using telehealth technology and electronic medical records.
• Ability to work independently in a remote environment.
