Job Overview
The Director of Care Coordination is responsible for supervising and supporting therapeutic care, treatment, and rehabilitation services across all care coordination programs within the region. This position provides leadership for care coordination and recovery support services, oversees program operations, supervises staff, manages grant-related reporting requirements, and ensures compliance with all applicable regulations, policies, and procedures. The Director may maintain an independent caseload, provide intake and assessment services, assist with referrals and aftercare planning, and support staff in delivering person-centered services. The role requires significant initiative, independent judgment, and leadership to achieve program goals.
Qualifications: Bachelor's degree (Master's degree preferred) in a recognized clinical discipline, Human Services, or a related field. Specialized training or certification relevant to the population served is preferred.
Experience: Minimum of four (4) years of experience in behavioral health counseling, care coordination, case management, or a related field.
Essential Functions, Duties & Responsibilities
Program Leadership & Operations
- Plan, implement, and oversee the CCBHC Care Coordination Scope of Care using an innovative, integrated, patient-centered approach.
- Develop and manage project plans, timelines, and schedules to ensure deadlines are met.
- Serve as a primary point of contact for program communications and stakeholder engagement.
- Ensure all program development initiatives align with the organization's mission, vision, values, and strategic objectives.
- Participate in internal and external CCBHC workgroups and collaborative initiatives.
- Develop implementation plans, milestones, processes, and performance measures for program initiatives.
- Identify and resolve complex operational and clinical challenges.
- Establish program goals and monitor progress toward objectives and timelines.
- Implement quality improvement strategies and evaluate outcomes to support continuous program enhancement.
- Provide detailed and accurate reports and presentations to senior leadership.
Care Coordination & Clinical Oversight
- Maintain current knowledge of community resources for individuals with substance use disorders, opioid use disorders, and mental health diagnoses.
- Ensure individuals receive trauma-informed, compassionate, and clinically appropriate services consistent with CCBHC requirements.
- Monitor admissions, discharges, referrals, alerts, and follow-up services.
- Ensure timely follow-up and coordination of care for individuals at risk of suicide or overdose following hospital or facility discharge.
- Ensure ADT (Admission, Discharge, Transfer) notifications are appropriately managed.
- Track referral trends, alerts, and program deliverables.
- Support interdisciplinary collaboration to improve client outcomes.
Financial & Revenue Expertise
- Medicaid and managed care reimbursement
- Productivity and utilization management
- (Sustainability planning is good but not required. We can assist with this across the agency.)
Staff Supervision & Development
- Hire, onboard, train, supervise, monitor, and evaluate Care Coordination and Recovery Support Services staff.
- Support the professional development and ongoing training of team members.
- Provide consultation, coaching, and intervention supervision to Care Coordination teams.
- Promote positive working relationships and collaboration across departments.
- Develop performance standards and evaluation measures to ensure high-quality service delivery.
- Conduct routine audits and reviews of services and program documentation.
Grant Management & Compliance
- Oversee grant-funded Care Coordination and Recovery Support Service programs.
- Provide leadership and supervision for BHSB and Anne Arundel County Care Coordination grants.
- Support Harm Reduction and Syringe Services Programs operating in Baltimore City, Baltimore County, and Anne Arundel County.
- Ensure compliance with grant requirements, licensing standards, ethical guidelines, and regulatory expectations.
- Prepare monthly, quarterly, and other required program reports.
- Collaborate with Division Directors and other stakeholders to ensure accurate and timely data collection and reporting.
- Review financial invoicing and reporting requirements related to grant-funded services.
Community & Partner Relations
- Maintain positive relationships with community partners and service providers.
Physical & Environment Demands
- Ability to sit, crouch, walk, and stand for extended periods (4-8 hours per workday)
- Ability to frequently drive and travel between worksites
- Capacity to stand, walk, and move within clinical or community settings throughout the workday
- Frequent use of hands and fingers for typing, writing, and operating computers, phones, and EHR systems
- Ability to see, hear, and speak clearly for effective communication with staff, clients, families, and care teams
- Occasional need to lift or carry light objects (generally up to 10–20 pounds), such as files, laptops, or educational materials
- Ability to maintain focus and mental alertness during emotionally intense or crisis situations
- Capacity to perform duties in fast-paced, high-stress environments, including crisis intervention settings
- May be required to respond physically and verbally during emergencies to ensure client and staff safety
- Tolerance for exposure to emotionally challenging situations, including clients experiencing acute distress, trauma, or behavioral instability.

Pay: $71,684.00 - $80,645.00 per year
