NURSE PRACTITIONER or PHYSICIAN ASSISTANT

SCHEDULE: Full-Time. Monday-Friday.

RESPONSIBILITIES:

1. Provide complete patient history, physical examinations, diagnosis, and plan for treatment of primary, secondary, and tertiary care services as outlined in, and in compliance with the standardized protocols for Nurse Practitioners/Physician Assistants.

Primary care delivery includes communicating with patients or guardians of patients regarding care plans, results of diagnostic studies, and answering patient and family questions. Also included is organizing and delivering health education for patients, including general health information and disease specific education. The primary care provider coordinates patient referrals for specialized health care to public health care agencies.

2. Establish and maintain a confidential medical record for each patient.

3. Organize and administrate delivery of services.

4. Communicate with Medical Director regarding quality care and treatment issues of patients.

5. Conduct Clinical Pertinence Chart Review studies in order to assess and enhance patient outcomes.

6. Write and revise policies and procedures for the mobile clinic and standardized protocols for Nurse Practitioners/Physician Assistants with the clinic administrator and the medical director.

7. Develop, manage, and maintain statistical and informative data as well as other corporate documentation, including licensing and patient information.

8. Coordinate clinic scheduling and site selection.

9. Assure that services are provided in accordance with specified policies and regulatory agency's policies.

10. Serves as a resource for community agencies.

11. Communicate with CEO, reporting on issues related to the functioning. Interact as appropriate with partner agencies.

12. Computer use for research, data entry, record keeping, and business communication via email.

QUALIFICATIONS:

1. Valid California drivers’ license.

2. Skilled in EMR.

3. Family Nurse Practitioner with FQHC experience, preferred.

4. Bilingual in Spanish, preferred.

TRAINING, EDUCATION AND LICENSURE

1. Masters Degree or higher.

2. Current BLS certification.

3. A valid license to practice as a Registered Nurse in the State of California.

4. A certificate of completion from an accredited nurse practitioner/physician assistant program.

5. Training and experience should be in Family Medicine and ability to see all ages- pediatric to geriatric.

Compensation: $135,000-$180,000 per year depending on experience plus benefits.
