Job Summary

We are seeking a compassionate and dedicated Certified Nursing Assistant, BHT/RADT, or LVN to join our dynamic healthcare team. In this vital role, you will provide high-quality, patient-centered care to individuals across various settings and levels of care. Duties

• Daily medication management for census of up to twelve.
• Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring proper documentation within electronic health records (EHR) systems.
• Support medication administration under supervision, ensuring compliance with HIPAA regulations and maintaining patient confidentiality at all times.
• Provide specialized care for patients who require additional attention outside of standard medication protocols.
• Daily nursing notes and progress reports for each patient. Assisting RN with documentation. Intake assessments and discharge planning.
• Maintain a safe environment by practicing safe lifting practices during patient transfers and heavy lifting tasks while following infection control protocols.
• Collaborate effectively with multidisciplinary teams in sub-acute care environments as well as outpatient.

Skills

• Proven experience working with SUD and MH individuals and other behavioral health needs.
• Familiarity with EMR/EHR systems such as KIPU for accurate medical documentation and record management.
• Strong knowledge of medical terminology, anatomy, physiology, and basic life support (BLS) procedures like CPR.
• Ability to perform manual handling safely while adhering to OSHA standards for patient transfer methods and heavy lifting.
• Excellent caregiver communication skills to effectively coordinate with patients, families, and healthcare professionals.
• Experience with DHCS and Joint Commission standards.
• Experience working within SUD and MH population.
