About Addiction Treament Services

NYC Health + Hospitals is at the forefront of delivering innovative substance use disorder (SUD) treatment services. Our emphasis is on providing access to culturally responsive, person-centered SUD care when and where it is needed, encompassing a holistic approach that includes social determinants of health. We deliver comprehensive substance use care for acute or complex needs, and training and education for both SUD and non-SUD staff to support the next generation of addiction champions. About the Role: Addiction Counselor, Level I

The Addiction Counselor, Level I serves as the primary clinician for the individuals with whom they work. In a sensitive and caring manner, they promote the notion of recovery and self-sufficiency, and lead the development and implementation of clinically sound, culturally relevant, and medically monitored treatment programs, case management, care coordination and other supportive and consultative services for patients in SUD treatment programs. This role participates in and/or performs functions related to counseling, diagnostics, treatment, treatment planning, rehabilitation, assessment, intervention and other supportive services for patients in SUD treatment, and ensures optimal utilization of resources and compliance with all applicable local, state, and federal rules and regulations and NYC Health + Hospitals standards of care.

Example of Typical Tasks:

• Conducts initial screenings and assessments of patients; performs on-going toxicology screenings to obtain pertinent information about Substance Use Disorder (SUD) and Co-Occurring Disorder(s).
• Develops psycho-social histories; formulates appropriate diagnostic impressions; prepares accurate and timely screening, intake and assessment reports.
• Facilitates and/or co-facilitates individual, group, and family counseling; provides psychoeducation; conducts skills training.
• Makes decisions and maintains required records regarding appropriate Level of Care, at point of entry and on-going, using patient criteria tools (e.g. LOCADTR) and clinical judgement, to ensure patients are placed in the most clinically appropriate level of care.
• Develops and continually monitors and updates treatment/recovery plans in collaboration with patients and the clinical treatment team, including short- and long-term treatment goals, objectives, and interventions; accurately and timely records treatment planning consistent with established standards and regulatory requirements.
• Coordinates and provides case and care management services and activities; makes referrals and linkages.
• Initiates and finalizes discharge and transition planning in collaboration with patient and clinical treatment team; may make field visits and/or accompany patients to community services.
• Maintains timely, accurate, and appropriate documentation in compliance with all applicable standards to ensure appropriate reimbursement.
• Provides orientation to new patients, patients' families and significant others; discusses financial responsibility with patients and provides money management assistance.
• Provides crisis intervention and support by responding to and following through in crisis situations; demonstrates good clinical judgement by seeking additional assistance and/or consultation when needed.
• Conducts outreach activities, provides advocacy, and acts as a liaison between programs, providers and community supports.
• Participates in relevant staff meetings, case conferences and trainings; participates in special projects and internal/external quality assurance/performance improvement activities.
• Participates in community outreach activities on and off campus.
• Holds and maintains required certification(s) and completes required continuing education; complies with NYC Health + Hospitals policies and procedures and all applicable local, state and national requirements and guidelines.

Minimum Qualifications

1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Level 1 Certificate issued by the NYS Office of Addiction Services and Supports (OASAS); or

2. Is otherwise considered a Qualified Health Professional (QHP) based on licensure, certification and/or registration along with appropriate experience and/or training as defined in Section 800.4 (U) of the OASAS Treatment Services: General Provisions, Title 14 NYCRR Part 800 (https://oasas.ny.gov/system/files/documents/2022/09/800.pdf).

Knowledge, Skills, Abilities and Other Requirements

• Proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment (MAT)" training required.
• Knowledge of Substance Use Disorders treatment.
• Knowledge of Mental Health Disorders treatment.
