Pre-Release Coordinator
Location: Hybrid (Remote with occasional in-person meetings based on operational needs)
Reports To: Clinical Operations Supervisor
Compensation: $55,000–$68,000 annually

The Role
Leaving incarceration is one of the most vulnerable transitions a person can experience.
As a Pre-Release Coordinator, you'll help individuals prepare for that transition by making sure they aren't leaving with only uncertainty—but with appointments scheduled, services connected, resources identified, and a plan they understand.
Working primarily by phone and video, you'll become a trusted point of contact for participants before release. You'll coordinate healthcare services, connect people to community resources, communicate with correctional partners and care teams, and help ensure nothing falls through the cracks before someone returns home.
This is an operational coordination role—not a clinical position. You'll spend your time building relationships, solving problems, organizing complex moving pieces, and helping multiple organizations work together around each participant.

Work Environment
This position primarily provides participant engagement virtually through phone and video.
This is a primarily remote position, with occasional in-office work for team meetings, training, collaboration, or operational needs. Travel to correctional facilities is not a part of this role.
Because release dates, participant needs, and community resources change frequently, this position requires flexibility, adaptability, and strong communication skills.
