Schedule: 8-hour shift; 40 hrs./wk. (typically 8a-4p M-F)
FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is seeking a Registered Nurse Case Manager to work with patients, families, and other professionals to promote the philosophy of palliative care and the hospice concept. This position manages an assigned caseload of patients in our service area providing nursing care under the direction of the Hospice physician and in accordance with the patient’s individualized plan of care.
Our RNs have a pivotal role in the hospice goals and daily care of our patients. FAIRHOPE looks to its RNs to apply appropriate preventative, therapeutic, and rehabilitative nursing care to foster the best possible experience for our hospice patients, their loved ones, and caregivers.

Responsibilities:
- Performs preventative, therapeutic, and rehabilitative nursing practices and procedures.
- Coordinates patient care provision and handles incoming calls.
- Assesses the physical, mental, environmental, and security needs of the patient to ensure FAIRHOPE Palliative Care services are of high quality.
- Reviews and implements the patient's plan of care and makes necessary revisions.
- Administers medications and treatments as recommended by the physician according to nursing standards.
- Maintains accurate records of medication reconciliations and controlled substances.
- Participates in interdisciplinary groups to discuss patient care plans and issue lists.
- Completes documentation such as assessments, problems, goals, interventions, and patient outcomes in a timely manner.
- Assists the patient, family members, and caregivers in administering their care with the help of a healthcare team.
- Educates and informs the patients, family, and caregivers about relevant diagnosis and services.
- Follows HIPAA guidelines at all times.
Qualifications:
- Graduate of an accredited school of professional nursing, with a valid/current RN license from the Ohio Board of Nursing
- Possess current Basic Life Support (BLS) certification
- Three years of recent nursing experience; end-of-life care and/or healthcare setting knowledge is preferred
- An individual with the ability to identify areas for improvement and take necessary action
- Demonstration of excellent communication and interpersonal skills, as well as a caring attitude
- Working knowledge of computers
- Physical Setting: Various healthcare settings. Some driving is required.
- Work Location: Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties. Main office location is located in Lancaster, Ohio.
