Duties

• Conduct thorough patient assessments, including vital signs, physiology evaluation, and medical history review to develop personalized care plans.
• Manage post-acute care and discharge planning for patients transitioning from hospital or inpatient settings to home environments.
• Provide skilled nursing services such as wound care, medication management, IV therapy, and infectious disease care within the home setting.
• Collaborate closely with healthcare professionals including physicians, therapists, and case managers to coordinate patient care and optimize health outcomes.
• Utilize Electronic Health Records (EHR) systems for accurate documentation of medical records, patient progress notes, and treatment plans while adhering to HIPAA regulations.
• Perform diagnostic evaluations and manage complex cases involving geriatrics, behavioral health, pediatrics, or patients with chronic conditions.
• Educate patients and their families on health management strategies, medication adherence, and safety protocols to promote independence and well-being.

Qualifications

• Valid RN license with current registration; additional certifications in critical care or trauma center experience are highly desirable.
• Proven experience in hospital medicine, ICU settings, emergency medicine, or Level I/II Trauma Centers demonstrating advanced clinical skills.
• Strong knowledge of medical terminology, physiology, infection control policies, and health regulation standards.
• Experience with EMR/EHR systems for electronic health records management; familiarity with hospice care and discharge planning is a plus.
• Ability to perform comprehensive patient assessments and manage complex cases across diverse populations including pediatrics and geriatrics.
• Excellent client communication skills with a focus on compassionate patient service and education.
• Demonstrated ability to collaborate effectively with healthcare professionals across inpatient and outpatient settings; managed care or utilization management experience is advantageous.

Pay: $136.48 - $143.94 per hour
