Registered Nurse Care Manager (RN)

About the Position

Clarkston Internal Medicine is seeking a compassionate, organized, and motivated Registered Nurse Care Manager (RN) to join our primary care team. This role focuses on supporting patients with chronic conditions, coordinating care transitions, improving preventative health outcomes, and helping patients navigate the healthcare system.

The ideal candidate is detail-oriented, comfortable communicating with patients by phone and in person, and passionate about improving patient outcomes through education, follow-up, and care coordination.

Responsibilities

• Coordinate care for patients with chronic and complex medical conditions
• Perform Transitional Care Management (TCM) outreach and documentation
• Review hospital discharges, ER visits, and specialist recommendations
• Collaborate with providers, specialists, hospitals, home care agencies, and community resources
• Educate patients on medications, treatment plans, and disease management
• Monitor and document patient progress within the EMR
• Assist with care plans and patient goal setting
• Communicate professionally with patients, families, and healthcare teams
• Support population health initiatives and value-based care programs

Qualifications

• Active Michigan Registered Nurse (RN) license — required
• Minimum 2 years of clinical nursing experience preferred
• Primary care, case management, care coordination, or chronic care management experience preferred
• Strong organizational and communication skills
• Ability to work independently while collaborating with a team
• EMR/Epic experience preferred
• Knowledge of Medicare, Transitional Care Management, and preventative care workflows is a plus
