Job Description:

Seeking an experienced Registered Nurse (RN) for an Adult Home Care assignment in Manhattan. This is a true home care position requiring nurses to provide direct patient care in patients' homes. The RN will manage approximately 4–8 patient visits per day and collaborate with the interdisciplinary team to ensure high-quality, patient-centered care. Lyft transportation assistance will be available within the assigned borough.

Responsibilities:

• Assess, plan, implement, and evaluate individualized patient care plans.
• Provide direct nursing care in patients' homes.
• Perform comprehensive physical assessments and monitor patient conditions.
• Administer prescribed medications and document patient responses.
• Initiate and maintain IV therapy as ordered.
• Perform wound care and adhere to infection control protocols.
• Change dressings, insert catheters, and operate specialized medical equipment as needed.
• Monitor vital signs and promptly report adverse reactions or changes in patient status.
• Educate patients and families regarding treatment plans, medications, and post-operative care.
• Collaborate with physicians and interdisciplinary teams on discharge planning and ongoing care needs.
• Maintain accurate and timely documentation in patient medical records.
• Follow established nursing standards, policies, and best practices.

Required Qualifications:

• Active New York State Registered Nurse (RN) License with Primary Source Verification.
• Associate Degree in Nursing from an accredited institution.
• Current AHA Basic Life Support (BLS) certification
• Current AHA ACLS certification
• Minimum two (2) years of recent Med/Surg experience in: A large acute care hospital setting, or A post-acute care facility.
• Most recent Med/Surg experience must be within the past year.
• Previous experience providing nursing care in patients' homes.

• Physical assessment
• Medication administration
• Wound care and infection control
• IV therapy
• Post-operative care
• Patient education
• Telemetry monitoring

Preferred Qualifications:

• Experience using EPIC EMR.

Additional Information:

• Home Care experience is required.
• Approximately 4–8 patient visits per day.
• Every other weekend availability is mandatory.
• Lyft rides will be provided within the assigned borough.

Pay: $95.00 - $100.00 per hour
