About the Role: Medical Case Manager

Kumukahi Health + Wellness is seeking a compassionate and motivated Medical Case Manager to join our West Hawaiʻi team. In this role, you will work directly with individuals living with HIV, helping them navigate healthcare systems, access vital resources, and remain engaged in medical care.

As a Medical Case Manager, you will play a critical role in improving health outcomes, reducing barriers to care, and empowering clients to achieve their personal health goals. You'll work alongside a dedicated multidisciplinary team committed to providing client-centered, culturally responsive care.

What You’ll Do:

• Conduct comprehensive assessments and develop individualized care plans.
• Coordinate medical, behavioral health, and supportive services for clients living with HIV.
• Assist clients in accessing medical appointments, medications, diagnostic testing, and community resources.
• Collaborate with healthcare providers, social service agencies, and community partners to ensure coordinated care.
• Provide education on HIV treatment, prevention, medication adherence, and overall wellness.
• Support clients in addressing barriers related to housing, transportation, insurance, and financial assistance.
• Advocate for clients and empower them to make informed healthcare decisions.
• Maintain accurate, timely, and confidential documentation in electronic health record systems.

What We’re Looking For:

Minimum Qualifications:

• Bachelor’s degree in Nursing, Social Work, Public Health, or a related field preferred.
• One to three years of experience in case management or healthcare, ideally with a focus on HIV care.
• An equivalent combination of education and experience may be considered.
• Strong communication skills and a compassionate, nonjudgmental approach.
• Knowledge of HIPAA regulations and patient confidentiality.

Preferred Qualifications:

• Experience working with individuals living with HIV or other chronic conditions.
• Certification as a Case Manager (CCM) or related credential.
• Familiarity with EHR systems and case management software.

Pay Range: $21 - $23 per hour
