Summary:
The Remote Patient Monitoring Technician (RPM Tech) supports The Guthrie Clinic’s Remote Patient Monitoring program with successful patient transitions from hospital to home, chronic disease management, care coordination, and patient triage. The RPM Tech partners with the Care Transitions Nurse Navigators and other RPM-associated team members. Responsible for linking mobile resources together, integrating and coordinating them to allow for an appropriate response to patient needs. Telephonic patient triage is provided following established evidence-based protocols to assist in navigating care across the health care continuum. Demonstrates appropriate communication skills and interactions with patients, family members, and other members of the patient care team. Assists with the organization and completion of simple projects under the supervision of Pulse Center leadership.
Experience:
Prior clinical or direct patient care experience preferred but not required. Proficiency with Microsoft programs (Excel, Word, Outlook, Teams, etc.) preferred. Professional communication and teamwork required.
Education:
High School education or GED required. Must be comfortable using computers and learning new technology.
