Job Summary
Hinds LifeCare is hiring a Hospice Inpatient Hospital Liaison to conduct clinical evaluations, coordinate hospice admissions, and support strong hospital and facility partnerships across Fresno, Madera, and Merced counties.
This role reports directly to the Director of Hospice Access and Inpatient Services. In the liaison capacity, the RN collaborates with hospitals, facilities, and other healthcare partners to determine hospice eligibility and complete admissions for both inpatient and outpatient services under the supervision of the Clinical Administrator. This position also works closely with the Patient Access team and Business Development/Outreach team to support seamless transitions into hospice care and strengthen referral source relationships.
Key Responsibilities
- Conduct clinical evaluations in collaboration with hospitals, facilities, and healthcare partners to determine hospice eligibility
- Complete hospice admissions for both inpatient and outpatient services
- Respond promptly to requests for clinical evaluations and informational meetings from referral sources, patients, and families
- Explain hospice philosophy, hospice services, and levels of care, including General Inpatient (GIP), with clarity and expertise
- Assess patient and family needs, goals, and readiness to support appropriate hospice placement
- Coordinate with interdisciplinary team members and the Medical Director to expedite admission decisions
- Complete informed consent with patients and families accurately, compassionately, and in accordance with regulatory standards
- Represent Hinds LifeCare professionally within hospitals and facilities
- Provide excellent customer service and communication to maintain and strengthen referral relationships
- Collaborate with the Director of Inpatient Hospice Services to support and maintain census goals
- Support smooth transitions into hospice care through strong coordination, responsiveness, and follow-through
Required Qualifications
- Completion of a professional nursing program accredited by the Board of Registered Nursing
- Bachelor’s degree in Nursing required
- Master’s degree preferred
- Current RN licensure
- Minimum 3 years of nursing experience in a hospice or palliative care setting
- Strong clinical assessment, critical thinking, and independent decision-making skills
- Ability to work effectively with interdisciplinary healthcare team members and referral partners
- Knowledge of hospice philosophy, hospice levels of care, and end-of-life care coordination
- Sensitivity to cultural differences and ability to provide compassionate, patient-centered communication
- Knowledge of current trends in home health care, federal and state regulations, state licensing requirements, and related healthcare standards
- Current CPR certification
- Recent physical and required health clearances, including TB, Tdap, MMR, Varicella, and HepB immunizations
- Valid driver’s license and willingness to use a personal vehicle in the course of employment
- Proof of automobile insurance, current registration, and acceptable driving record with no more than 3 points within a 36-month period
- Ability to meet the physical requirements of the role, including pushing, pulling, and lifting/carrying up to 35 lbs
Preferred Qualifications
- 2 years of leadership experience in a hospice environment
- Certification in Hospice and Palliative Nursing
- Experience with inpatient hospice admissions, hospital liaison work, or referral coordination
- Experience collaborating with acute care hospitals, skilled nursing facilities, and other referral sources
Compensation and Benefits
Compensation: $125,000 - $128,000 annually
