We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to step into the role of Charge Nurse, leading nursing teams and ensuring exceptional patient care across diverse healthcare settings. Responsibilities

• Lead and coordinate nursing staff to ensure efficient delivery of patient care in accordance with facility policies, long-term care regulations, and best practices.
• Oversee patient assessments, vital signs monitoring, and physical examinations to develop personalized care plans.
• Administer medications, injections, IV infusions, and perform catheter care following strict protocols and medical documentation standards.
• Manage complex clinical procedures such as tube feeding, sterile processing, wound suturing, and airway management.
• Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and electronic health records (EHR/EHR systems).
• Conduct triage assessments for incoming patients or residents, prioritizing urgent needs such as acute pain management or infectious disease care.
• Collaborate with interdisciplinary teams including physicians, therapists, case managers, and family members to coordinate discharge planning and post-acute care.
• Utilize Point Click Care EMR system to document patient care accurately and efficiently.
• Provide mentorship and supervision to nursing staff while fostering a culture of continuous improvement and professional development.

Experience

• Valid Licensed Practical Nurse (LPN) license with current certification in CPR/BLS.
• Demonstrated critical care experience in settings such as ICU, emergency room, or trauma centers is highly preferred.
• Extensive knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and medication administration protocols.
• Proven experience working with diverse populations including geriatrics, individuals with developmental disabilities, dementia or Alzheimer's disease, and memory care patients.
• Familiarity with Point Click Care EMR/EHR system; experience managing medical records digitally is advantageous.
• Prior experience in long-term care facilities such as skilled nursing facilities or assisted living communities is desirable.
• Strong background in acute pain management, infection control practices, ventilator management, and hospice or palliative care is beneficial.
• Ability to perform complex clinical procedures including venipuncture, phlebotomy, catheterization, sterile technique, and first aid.
• Excellent client communication skills with a focus on caregiver communication and patient assessment accuracy.
