Position Summary
The nurse is responsible for providing individualized comprehensive nursing care to clients within the VFHC service area. The nurse is also responsible for care management and coordination of other needed services. These services will be provided according to the VFHC policy. The nurse also covers for other RN staff as appropriate.
Essential Duties and Responsibilities A. Conducts the admission of the client to the company by: 1. Obtains a medical history, particularly as it relates to the present condition, from the client and family member(s)/caregivers. 2. Reviews clients medications for contraindications, side effects, and efficacy on an ongoing basis. 3. Evaluates physical, emotional and spiritual well being. 4. Evaluates the client, family member(s), care giver(s) and home situation to determine what health teaching will be required. 5. Evaluates the client environment to determine what assistance will be available from family members in caring for the client. 6. Evaluates the client condition and home situation to determine if the services of a home health aide, RD, etc. will be required and the frequency of this service.
B. Develop and implements a nursing care plan with the client and family/caregiver that utilizes.
C. Client / Family Teaching: 1. Teaches, supervises, and counsels the client and caregivers regarding the nursing care needs and other related problems / concerns of the client at home.
D. Communication and Record Keeping. Completes the plan of care, overview, and makes necessary revisions for nursing and home health aide care.2. Prepares visit notes as per home care agency policy and procedure.3. Coordinates services among all staff, professional and paraprofessional and other health care providers as necessary.4. Informs the physician and other personnel of changes in the client’s condition and needs through care conferences and written feedback, i.e. multipurpose communication forms.5. Performs and records home health aides supervisory visits.6. Closes charts of discharged clients per agency policy and procedure.7. Arrange visit coverage for his/her clients during nurse's absence
VI. Supervision and Leadership. A. Supervises and teaches home health aides who provide care to those patients whom the nurse care manages. B. Is responsible for the care given by the home health aide by: 1. Supervisory visits of home health aides are conducted per home care policy and procedure, i.e. every two weeks. 2. Participates in development and implementation of educational programs to address home care staff members’ needs.
C. Is responsible for the care given by an RN by: 1. Performing supervisory visits per policy every 30 days. 2. Reviews visit notes and adherence to plan of care.
VII. Personal and Professional Growth A. Continues to strive for improved nursing care by broadening and deepening nursing knowledge through formal education, or attendance at workshops, conferences, or active participation in professional and related organizations.
B. Professional career objectives are developed and implemented.
VIII. Contributes to Organizational Goals A. Participates actively and appropriately in: 1. Client care conferences at minimum of three (3) per month. 2. In-service programs B. Participates in volunteered and assigned committee meetings.
C. Carries out agency’s philosophy and objectives.
D. Participates in quality improvement and utilization reviews activities of the agency.
E. Completes other duties as assigned or requested.
IX. Work Environment / Physical Demands1. Will be required to drive to clients’ place of residence within the agency service area in a variety of Michigan weather conditions.
2. May be required to enter clients’ homes which are cluttered, unsanitary, or disheveled. 3. Not required to enter or stay in any unsafe environment.
4. May be required to lift up to 50 pounds unassisted and up to 325 pounds utilizing an assistive device related to patient care or supplies; walking, bending, and stretching.
5. Will be provided writing space, a telephone for use and documentation system.
6. Volumes of paperwork, government regulations and accreditation body requirements. 7. May be required to provide after-hours, week-end and/or holiday on-call coverage.
X. Safety Equipment: 1. Personal protective equipment will be provided as needed, i.e. goggles, gowns, face shields, gloves, masks, anti reflux respirator shield.

8. CPR certified
9. IV certified per agency protocol.
II. Qualifications and experience:
1. Registered Nurse licensed to practice in the state of Michigan 2. Preferred to have or be pursuing a baccalaureate degree, and have a minimum of one year nursing experience 3. Physically able to perform duties of a Registered Nurse. Possession of a Michigan driver’s license 5. Professional liability insurance recommended
