PRIMARY RESPONSIBLITY:
Transitioning Youth Specialist job involves working with young adults aging out of foster care to help young adults develop a comprehensive, holistic and measurable plan for aging out of foster care and to guide and support the young adults in their transition to self-sufficiency and living an independent meaningful life.

EXAMPLE OF ACTUAL FUNCTIONS:
Responsibilities include: encourage young adults to maintain continuous enrollment in the most appropriate educational setting, refer the young adult(s) to community job placement agencies, housing and other community resources available. Assist young adults with making positive adult connections and coordinating services.

Specific duties include but are not limited to:

• Assist and assess young adults with educational planning, career planning, daily living skills, money management i.e. Financial Literacy courses, self-care, and any other areas needed for self-sufficiency. Guide and connect them to services, programs and resources based on findings; submit funding request as needed.
• Document in FSFN all contacts, referrals, and efforts made with young adults and providers.
• Verifies accuracy, content and completeness of all funding requests (i.e. EFC/PESS and Aftercare requests).
• Process requests in a timely manner in accordance with Chapter 409 Florida Statutes and Florida Administrative Code.
• Ensure all available community resources have been exhausted before submitting funding requests.
• Work with young adults to link them to positive adult connections or mentor.
• Monitors progress and compliance with program requirements. Attend dependency court hearings and administrative hearings when necessary.
• Provides intervention as needed and if necessary, refer and link young adults to crisis counseling and other behavioral health and-or substance abuse services-interventions.
• Conducts home visits or facilitates visits/attendance to schools, camps or any other enriching activities geared towards self-sufficiency.
• Establish relationships with community agencies that will benefit the organization in carrying out its’ mission.
• Provide support and guidance to young adults with the goal of leading them to self sufficiency and leading a productive and meaningful life.
• Must develop expertise and working knowledge in the governing law, rule and polices of the Transitioning Youth Program Services.
• Available to provide on call, on the weekends, and/or holiday’s assistance to clients—on an as needed and appropriate basis, in rotation.

MINIMUM REQUIREMENTS:
Bachelor’s degree in education, social work or other related Human Services field—Master’s Degree is preferred. Knowledge of Transitioning Youth Services. Must be proficient in computer related programs.

SPECIAL SKILLS AND OTHER REQUIREMENTS:

• Possess a current Background Clearance Screening Letter as required by DCF.
• Local travel is required, a Valid Florida driver’s license and documentation of current automobile insurance is required.
• Successful completion of the Independent Living Certification within 90 days of hire date.
• Child Welfare Certification and field work should be obtained within 1 year of hire date.

Job Type: Full-time

Pay: $45,000.00 - $56,000.00 per year
