Position Summary

The Hospice Nurse Practitioner serves as the primary provider for an assigned geographic territory, delivering expert hospice and palliative care while serving as a trusted clinical resource for patients, families, facilities, and the interdisciplinary team.

This position combines direct patient care, clinical leadership, facility partnership development, and provider collaboration. Rather than functioning solely as an admissions provider or recertification provider, the Regional NP maintains an active presence in the field, building strong relationships with assigned facilities and supporting continuity of care throughout the hospice journey.

Primary Hospice Clinical Care Responsibilities

• Perform hospice admission evaluations and determine hospice eligibility
• Conduct Medicare Face-to-Face encounters for hospice recertification
• Evaluate and manage pain and complex end-of-life symptoms
• Prescribe medications and treatments within Oregon Nurse Practitioner scope of practice
• Perform routine provider follow-up visits for medically complex hospice patients
• Complete urgent symptom management visits when clinically indicated
• Collaborate closely with the Hospice Medical Director regarding patient care
• Collaborate closely with Hospice RNCM regarding patient care
• Participate in Interdisciplinary Group (IDG) meetings
• Assist with development and revision of individualized plans of care
• Maintain accurate, timely, and CMS-compliant documentation

Regional Territory Responsibilities

The Nurse Practitioner will be assigned a defined geographic territory and will serve as Everlight Hospice's primary clinical provider within that region.

Responsibilities include:

• Maintain a consistent clinical presence within assigned skilled nursing facilities, assisted living communities, adult foster homes, and memory care communities
• Develop long-term professional relationships with facility administrators, Directors of Nursing, providers, nurses, social workers, and caregivers
• Conduct routine provider rounds within assigned facilities to improve accessibility and continuity of care
• Serve as a trusted clinical consultant for facility staff managing hospice patients
• Participate in family meetings, care conferences, and complex goals-of-care discussions
• Provide education regarding hospice philosophy, symptom management, disease progression, and end-of-life care to patients, families and our community partners
• Identify opportunities to improve communication, responsiveness, and overall customer satisfaction within the assigned territory
• Partner with Everlight leadership to strengthen referral relationships through exceptional clinical service and professionalism

Collaboration & Leadership

• Support RN Case Managers with complex clinical decision-making
• Mentor and educate nursing staff regarding symptom management and hospice best practices
• Collaborate with admissions, nursing, social work, spiritual counselor, and therapy services to provide exceptional interdisciplinary care
• Assist with QAPI program (quality assessment and performance improvement) initiatives and clinical process development
• Promote Everlight Hospice's culture of compassion, responsiveness, and clinical excellence

Call Responsibilities

This position participates in a shared provider call rotation of approximately one week every three weeks.

During scheduled call weeks, responsibilities include:

• Telephone consultation and clinical decision-making
• Collaboration with the on-call nursing team
• Occasional evening, weekend, or holiday hospice admissions
• Occasional urgent symptom management visits when clinically necessary

The posted salary reflects the full scope of this position, including participation in the shared provider call rotation

Required Qualifications

• Current Oregon Nurse Practitioner license
• Oregon prescriptive authority
• Current DEA registration
• Current driver's license with reliable transportation
• Excellent clinical assessment and communication skills
• Ability to practice independently while collaborating within an interdisciplinary team

Preferred Qualifications

• Hospice or palliative care experience
• Experience in skilled nursing facilities, assisted living, or home-based care
• Strong leadership and relationship-building abilities
• Experience managing complex symptom burden and serious illness
