Job Overview
We are seeking a dedicated and experienced Full-Time Registered Physical Therapist to join our team at a reputable Skilled Nursing Facility. Duties

• Develop, implement, and modify individualized care plans based on patient assessments and treatment goals.
• Provide direct patient care across inpatient, outpatient, and home & community care settings, including pediatrics, geriatrics, orthopedics, and medically complex cases.
• Conduct evaluations utilizing physiology knowledge, anatomy expertise, and medical terminology to determine appropriate interventions.
• Manage discharge planning processes to ensure seamless transitions from hospital or inpatient settings to home or community environments.
• Utilize EMR systems efficiently for documentation, progress notes, and treatment updates.
• Perform therapeutic interventions such as physiotherapy techniques, trigger point therapy, and other modalities to promote recovery and pain relief.
• Educate patients and caregivers on exercises, mobility techniques, and health maintenance strategies.
• Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes.
• Maintain compliance with healthcare regulations and safety standards while delivering high-quality patient care.

Experience

• Valid registration as a Physical Therapist with current licensure in the state.
• Proven experience in acute care hospitals, skilled nursing facilities, outpatient clinics, or home health environments.
• Demonstrated expertise in care plans development for diverse populations including pediatrics and geriatrics.
• Familiarity with EMR systems for documentation and patient management.
• Strong background in physiotherapy practices such as orthopedics, inpatient rehabilitation, outpatient therapy, trigger point therapy, and community care programs.
• Knowledge of medical terminology related to musculoskeletal, neurological, and cardiovascular conditions.
• Prior experience with discharge planning processes and working within multidisciplinary teams is highly desirable. Pay: $48.00 - $55.00 per hour
