Support Solutions is seeking a full-time Service Coordinator to join our Residential Services team. This position is based in our Lewiston, Maine office and plays an important role in supporting residential programs, documentation compliance, service planning, and communication across teams.

The Service Coordinator works under the direction of the Director of Care Management and provides coordination, guidance, training, and compliance support related to Person Centered Plans, Individual Service Plans, Reportable Events, documentation, and regulatory requirements.

This is a strong fit for someone who is organized, professional, detail-oriented, and passionate about supporting individuals with intellectual and developmental disabilities through high-quality service coordination.

What You’ll Do

The Service Coordinator helps ensure that residential services remain person-centered, organized, compliant, and well-documented.

Responsibilities may include:

• Supporting oversight of Person Centered Plans and Individual Service Plans
• Tracking PCPs, ISPs, annual documents, deadlines, and required follow-up items
• Coordinating and participating in service planning meetings with individuals, guardians, case managers, residential staff, and other team members
• Writing or revising ISP goals, narratives, MaineCare home support service descriptions, and related documentation
• Supporting Residential Coordinators and staff with documentation expectations
• Assisting with intake documentation, electronic files, Therap entries, and required records
• Reviewing Reportable Event documentation for accuracy, completeness, and timely submission
• Tracking trends, concerns, and follow-up needs related to Reportable Events
• Supporting HCBS compliance, program licensing requirements, and agency standards
• Maintaining timely, professional communication with managers, guardians, case managers, external providers, and internal team members
• Participating in the agency’s on-call supervisor rotation

What We’re Looking For

The ideal candidate will be dependable, organized, and able to manage multiple priorities while maintaining strong attention to detail. This role requires professionalism, confidentiality, strong written communication, and the ability to support others with clarity and respect.

Qualifications include:

• High school diploma or GED required
• One to three years of experience in residential services, case management, human services, healthcare, compliance, or a related field preferred
• Experience with person-centered planning, ISP/PCP documentation, MaineCare services, or residential programming preferred
• Proficiency with Microsoft Office required
• Experience with Therap, Evergreen, Saddleback, or similar documentation systems preferred
• Valid driver’s license required
• Ability to maintain confidentiality and comply with agency, state, and federal documentation standards
• Strong organization, follow-through, written communication, and problem-solving skills
• Must be authorized to work in the USA

Compensation

$47,000 per year with opportunity to earn more
