This position will be specifically for Wright and Douglas Counties. Training will be required to be on-site at the Marshfield office. Upon completion of training there will be a requirement to work in the Marshfield office 2 times per week or more and the other days will be hybrid from home or conducting meetings in the communities. Travel is required and re-imbursed.

Duties

• Develop and implement personalized support plans tailored for clients, ensuring alignment with their goals and needs.
• Coordinate services across various agencies, including welfare, Medicaid, Section 8 housing, and workforce development programs.
• Conduct crisis intervention and crisis management to assist clients during urgent or challenging situations.
• Maintain detailed documentation of client interactions, service plans, and progress reports in compliance with organizational standards.
• Medicaid billing required.
