100 sponsored by employee, not by employer. Following ACA law so closely employee pays 9.6% of gross income for themself for a plan with $6K deductible. Add 9.6% for every family member you must cover. Only go here if you can afford that or you have coverage through another family member.

I like the PPO, but the coinsurance/copay seems kind of high
