Your vision benefits are provided through UnitedHealthcare (UHC) Spectera network, which provides coverage for preventive care, eyeglasses and contact lenses. UHC offers its vision program through a national Spectera Eyecare network, including both private practice and retail chain providers. UHC makes obtaining vision care quick and easy, giving you immediate access to benefits without any paperwork hassle. Although the Plan offers the best value when members use in-network providers, it also includes coverage for outof-network care. If you choose an out-of-network provider, you must pay the full amount at the time of service and submit a claim form along with a copy of your receipts and prescription to UnitedHealthcare. You will then be reimbursed directly by UnitedHealthcare, according to the schedule, up to the maximum allowance.
