Cigna medical has a low deductible, relatively low copays and covered 100% of two major surgeries while I was covered. Getting approval for some procedures can be difficult.

expensive premiums, unlike most tech companies, employee costs are not covered 100%. insurance provider has a bad reputation as being difficult to work with for doctors.
