It's Cigna and accepted by most providers. The company pays 100% toward the employees' portion. However, there is a waiting period so you don't get the benefits right away.

I didn't use it much (if at all?), actually. I was under the age of 26 whilst working there, and was covered under my dad's company insurance. IIRC, it was like $60-$90/mo. for HMO-PPO options, respectively.
