Supplemental Life Insurance

Voluntary Employee Life:

Benefit Amount: 5X annual salary, up to $500,000, in increments of $10,000

Maximum: Increments of $10,000 to $500,000 up to 5X annual salary. In no event shall life insurance benefits exceed five times your annual salary.

Guaranteed Issue: 5x salary, up to $150,000

 

Voluntary Spouse Life:

Benefit Amount: 50% of Employee's Benefit, up to $50,000, in $5,000 increments

Maximum: 50% of employee's benefit up to $50,000

Guaranteed Issue: 100% of Employee's Benefit, up to $25,000

 

Voluntary Child Life:

Benefit Amount: Increments of $1,000 up to $10,000

Maximum: $10,000

Guaranteed Issue: $10,000
