Very limited, no flexibility, no sick leave. People come to work sick because they don’t want to use PTO for sick days.

3 weeks PTO at hire and then 4 weeks at year 5 - they are very flexible about working from home or making up time if needed for appointments or childcare etc

You do not with any PTO. Once you do accrue it you must use it for sick days also and it is gone very quickly. Even if you are home with a sick child and you could be working from home they want you to take PTO.
