Two plans, Blue Cross Anthem (PPO and HMO), and Kaiser. I paid 250 per month for me, my wife, and my 5 year old son. After I was laid off, they gave me the insurance for 6 months at the same rate, then Cobra kicked in and it was more than 1 grand per month (prohibitively expensive).
