I only pay $80 a month for United Healthcare's top plan. They also have HSA available as well.

I had no major issues with coverage the one time I needed to use it. Had I not been covered the cost would have been a major setback to my financial situation.

The HDHP with HSA option is a good option for younger/healthy employees as it lets you plan for your future. They have a company contribution of around $600 each year, which is nice. It had been $1000 when I started, which was great. But they lowered it for FY2019.
The HSA has great fund options for low-cost index investing.
