VOLUNTARY/SUPPLEMENTAL LIFE INSURANCE (Employee Paid)

  • Employees can purchase supplemental life insurance in $10,000 increments, from $10,000 up to the lesser of $400,000 or 5x annual earnings. A Statement of Health is required if the election exceeds 3x annual salary (or $150,000) or if the employee enrolls after initial eligibility. Employees who elect supplemental coverage may also elect Voluntary Spousal, Dependent, or Accidental Death and Dismemberment coverage.

  • Spousal coverage is available in $5,000 increments, up to $50,000. The Guaranteed Issue amount for a spouse is the lesser of 50% of the employee's election or $20,000; a Statement of Health is required for higher amounts.

  • Dependent child coverage (under age 26) is available at $10,000 per child.
