It is really not affordable for your salary. They do offer health, dental, vision, life and 401K. The life, dental and vision are reasonable.

Health Insurance is only through Kaiser Permanente, only health and vision insurance. They used to have bonuses when you reach certain adherence levels but since discontinued all bonuses without notifying anyone prior.
