Available to full-time employees, and part-time employees requisitioned 40 hours per pay period. Employees are eligible on the first day of the month following 30 days of continuous employment, or during open enrollment. The benefit includes a co-pay for an eye examination every 12 months, a co-pay for lenses every 12 months and frames every 24 months. There are two plans to choose from for provider networks.
