Worst health insurance I've ever seen (and I worked at a benefits company previously). DISH is self-funded for health insurance so they do whatever they can to keep costs low. The plan complies with the ACA, but I'm not sure how because no part of the plan is "affordable" except for the monthly premiums which do NO good until you pay a $5k deductible.
