In Iowa, the Point of Service plan has no deductible. For hospital admissions the copay is only $250 then the plan pays 100% after that. Generic drugs are $5. The monthly premium is $87.32 and it covers medical, dental and vision, and that's before wellness incentives (can get up to $55/mo off the health ins. premium).

Very good, can include self, spouse and dependents for a very reasonable cost to the employee for great coverage. Includes vision and dental insurance as well.
