Form Factor has two options for dental coverage, Delta Dental PPO and Willamette Dental HMO
Under Delta Dental, you have access to the Delta Dental PPO Network, Out of Network Dentists and Premier Network dentists. Your out of pocket costs will be lowest if your dentist is a Delta Dental PPO network provider. Services for Premier Dentists are available at the in-network benefit level, but the discounts are not as high as with PPO network dentists, so your out of pocket costs may be significantly higher.
Willamette Dental HMO (Oregon and Southwest Washington only)
FormFactor offers HMO dental coverage through Willamette for employees who reside in Oregon and Southwest Washington. Under Willamette, you must receive care from a Willamette Dental Group dentist or specialist. This program currently has no deductibles or annual maximums. The program has set copays for each provided service including comprehensive orthodontia coverage for all ages.
