You choose the single or family option. It's interesting how HDR sets it up - everyone pays one flat monthly cost (e.g., for family it's $21.70/month), then there's a "cost sharing" multiplier based on your salary (multiplier is 0.145% as of 2018). That way everyone pays their fair share and people with lower salaries are unduly burdened by the shared costs.
Preventative costs are free (two exams/cleanings per year, one of which includes x-rays, plus additional things for kids like fluoride and sealants). Other items have reasonable (low) costs. They are divided into Type B (Maintenance Services), which are covered at 80% after deductible is met - these include extractions, root canals, fillings, etc.; Type C (Restorative Services), which are covered at 50% after deductible is met - these include crowns, bridges, dentures, replacement of prosthetics once every five years, etc.; and Type D (Orthodontic Services), which are also covered at 50% after deductible is met - things like braces.
