They have a yearly check that is voluntary. For example they check your blood pressure, weight, diabetes and more . If you meet 4 out the 8 criteria you get a discount. If you don’t meet the criteria you still get the discount by following up with your primary doctor.

Ok coverage with very expensive weekly contributions.

Avoid the high deductible plan at all costs. It is not worth it.

They are self insured, and the cost to have anything don is exorbitant for what the benefits cost. They insist that the cost is due to current politics, which seems a little slanted to me. Getting them to pay a spouses claims or a dependents claims is an exercise in futility, the continuously deny the claims for second party insurance, even after repeated calls to verify that there isn't any.
