Weird program that doesn't say up front that there is a minimum $250 "administrative fee" for dentists who are out of network, but that fee can be any amount and the dentist office can't tell you how much, it's not in any insurance paperwork, and even the agent who calculates the fee apparently can't say how much the fee is?
